Cartilage deterioration under kneecaps?

I was diagnosed with this condition when I was 12, and have since lived with it. I walk with a limp now because of it(age 23), and my knee has a tendency to pop out of place under physical stress. When I was 12 we had a pamphlet on it, but I can't remember what it's called, nor can my mother. Any help?
